Please read the following FAQ’s for answers to common questions.
1) How do I register on the state website?
You must register using a laptop or a PC. You can not register using a mobile device.
Go to https://www.health.pa.gov/Pages/default.aspx
Click on “Apply For ID Card/Access Account”
Click on “Adult Patient Registration”
Complete online form with your information. It is recommended that you capitalize everything. Have your valid Pennsylvania driver’s license or state-issued ID (or change of address card if applicable) in front of you while registering as you will need to match the information exactly as it appears on the ID. Make sure that the information you are entering matches with PennDOT records if you have updated your address. Enter only the digits where numbers are required (no spaces, hyphens, etc.)
After your information is validated, you will be prompted to create a username and password.
2) Can I register online before I see my doctor?
Yes, you should register and have your patient ID number prior to seeing an approved doctor.
3) How much does the doctor visit cost?
Medical Evaluations at Nature’s Pain Management costs $199. We accept cash or credit cards.
4) What happens after I am certified by a doctor?
After the doctor certifies you in the system, you will receive an email confirmation with your next steps on how to order your medical ID card.
5) How do I order my patient ID card?
Log in as a returning user to the PA DOH website with the username and password that you created when you registered. Usernames and passwords are case-sensitive. Click the ‘Make Payment’ tab and follow the prompts to enter payment information. The fee for the patient or caregiver ID card is $50. Individuals who participate in Medicaid, PACE, CHIP, SNAP and WIC may qualify for a discounted ($25) ID card. A confirmation will appear when you have successfully paid for the medical ID card.
6) When will I get my patient ID card?
You should receive your card approximately 10-14 days after you order it online. Your card will be mailed to the address registered with DOH website.
